When planning a special event a copy of the Risk Matrix Form (RMF) and Event Proposal must be submitted to the Office of Risk Management and Safety (RMS) at least thirty (30) days prior to the event.

Information provided within the RMF will be used to determine if additional risk controls or insurance should be implemented. The event will not need to be assessed for future dates unless the scope of the activities change.

At that time the event coordinator will need to submit a Special Event Application Form. An account number, estimated number of participants and signature must be included in order for the application to be processed. The event packet must be submitted to RMS at least twenty-five (25) days prior to the date of the event. The appropriate waiver form is provided below for your use.

You will be charged for the estimated number of participants, please plan accordingly.
