Requests to possess an ESA in a residential facilities including their common areas must be approved by the&nbsp;Office of Student Accessibility Services&nbsp;and Residence Life. Students must present a letter from a qualified professional licensed in Texas stating details about the patient\u2019s needs, treatment, and how the animal is a vital part of that treatment. The ESA must also have all their immunizations records from their veterinarian. ESA registration from an outside organization is not recognized.<br><\/p>\n<\/div><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">ESA Request Process<\/h2>\n\n\n\n<div class=\"wp-block-tarleton-blocks-instructions-block instructions-block layout-style-style2 has-texan-white-color has-tarleton-purple-background-color has-text-color has-background\" style=\"background-color:#f8f8f8;border-radius:50px\" role=\"region\" aria-label=\"Stacked Instruction Steps\">\n<div class=\"instructions-step--style2\"><div class=\"instructions-header\"><div class=\"instructions-number\" style=\"color:#4f2d7f;background-color:#ffffff;border-radius:50%;font-size:40px\"><span>1<\/span><\/div><\/div><div class=\"instructions-content style2-content\"><div class=\"instructions-heading\"><h2 style=\"color:inherit\">Request for Accommodations<\/h2><\/div>\n<p class=\"wp-block-paragraph\">Complete the Office of Student Accessibility Services\u2019 Request for Accommodations AIM <a href=\"https:\/\/kea.accessiblelearning.com\/Tarleton\" data-type=\"link\" data-id=\"https:\/\/kea.accessiblelearning.com\/Tarleton\" target=\"_blank\" rel=\"noreferrer noopener\">Application Form Link<\/a><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"instructions-step--style2\"><div class=\"instructions-header\"><div class=\"instructions-number\" style=\"color:#4f2d7f;background-color:#ffffff;border-radius:50%;font-size:40px\"><span>2<\/span><\/div><\/div><div class=\"instructions-content style2-content\"><div class=\"instructions-heading\"><h2 style=\"color:inherit\">Discuss ESA Process with Staff<\/h2><\/div>\n<p class=\"wp-block-paragraph\">Meet with the Office of Student Accessibility Services staff to discuss ESA process<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"instructions-step--style2\"><div class=\"instructions-header\"><div class=\"instructions-number\" style=\"color:#4f2d7f;background-color:#ffffff;border-radius:50%;font-size:40px\"><span>3<\/span><\/div><\/div><div class=\"instructions-content style2-content\"><div class=\"instructions-heading\"><h2 style=\"color:inherit\">Submit the following Required Documentation<\/h2><\/div>\n<p class=\"wp-block-paragraph\">Submit the provider letter to&nbsp;the Office of Student Accessibility Services<strong>&nbsp;<a href=\"mailto:studentaccessibilityservices@tarleton.edu\">studentaccessibilityservices@tarleton.edu<\/a><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A. <strong><strong><u>Letter from a Licensed Therapeutic Provider<\/u>&nbsp;(Letter must be no older than one year, written on professional letterhead, dated, and signed by the provider)<\/strong><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The need for the animal must be documented in a letter from a provider. For non-observable&nbsp;disability, documentation must be signed, dated, and presented on professional letterhead from a reliable and qualified professional. The letter provided by the therapeutic provider must contain:\n<ul class=\"wp-block-list\">\n<li>Whether the patient has a physical or mental impairment<\/li>\n\n\n\n<li>Whether the patient\u2019s impairment(s) substantially limit at least one major life activity or major bodily function<\/li>\n\n\n\n<li>Whether the patient needs the animal(s) (because it does work, provides assistance, or performs at least one task that benefits the patient because of his or her disability or because it provides therapeutic support to alleviate a symptom or effect of the disability of the patient, and not merely as a pet)<\/li>\n\n\n\n<li>A relationship or connection between the&nbsp;disability&nbsp;and the need for the animal<\/li>\n\n\n\n<li>Whether the health care professional has personal knowledge of the patient and a professional relationship with the patient\/client involving the provision of health care or disability related services<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Documentation obtained solely through an online service that provides emotional support animal documentation for a fee may not be considered a reliable source of information for purposes of evaluating the need for an emotional support animal as an accommodation. The university reserves the right to request additional documentation from a qualified treating provider who has personal knowledge of the student\u2019s condition and related accommodation needs.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">B. <strong><strong><u>Animal Health Information<\/u><\/strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Submit all Animal Health Information to&nbsp;The Department of Residence Life<\/strong>&nbsp;<strong>to Adrianna Rousseau at&nbsp;<a href=\"mailto:AROUSSEAU@tarleton.edu\">AROUSSEAU@tarleton.edu<\/a><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Proof of Vaccinations:<\/strong>&nbsp;Species specific&nbsp;vaccination and vet records to show proof of absence of fleas, ticks, or other parasites<\/li>\n\n\n\n<li><strong>Proof of Vet Prescribed Ongoing and Continuous Preventative:<\/strong>&nbsp;Species specific&nbsp;heartworm, flea, and tick preventative<\/li>\n\n\n\n<li><strong>Proof of spay\/neuter<\/strong><\/li>\n\n\n\n<li><strong>Color photo of the animal<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>The Department of Residence Life will follow up with the animal\u2019s veterinarian to verify that the animal does not exhibit aggressive or protective behaviors, is housebroken, and has reached an appropriate level of developmental maturity to reside in a campus housing environment.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">C. <strong><strong><span style=\"text-decoration: underline;\">Completion of Emotional Support Animal Documents<\/span><\/strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Submit&nbsp;Emotional Support Animal&nbsp;Guidelines and Documentation, Animal Owner Responsibilities, Animal Owner Responsibilities in Campus Housing Confirmation of Understanding Form and Roommate Acknowledgement Form to&nbsp;the Department of Residence Life<\/strong>&nbsp;<strong>to Adrianna Rousseau at&nbsp;<a href=\"mailto:AROUSSEAU@tarleton.edu\">AROUSSEAU@tarleton.edu<\/a><\/strong>.<\/p>\n\n\n\n<ol start=\"1\" style=\"list-style-type:lower-roman\" class=\"wp-block-list\">\n<li>ESA Guidelines and Animal Owner Responsibilities<\/li>\n\n\n\n<li>Animal Owner Responsibilities in Campus Housing Confirmation<\/li>\n\n\n\n<li>Roommate Agreement<\/li>\n\n\n\n<li>Animal Registration Form<\/li>\n<\/ol>\n<\/div><\/div>\n\n\n\n<div class=\"instructions-step--style2\"><div class=\"instructions-header\"><div class=\"instructions-number\" style=\"color:#4f2d7f;background-color:#ffffff;border-radius:50%;font-size:40px\"><span>4<\/span><\/div><\/div><div class=\"instructions-content style2-content\"><div class=\"instructions-heading\"><h2 style=\"color:inherit\">ESA Committe Review<\/h2><\/div>\n<p class=\"wp-block-paragraph\">Once all information from Step 3 has been submitted, the ESA Committee comprised of staff members of the Office of Student Accessibility Services and Department of Residence Life will review ESA applications. The ESA Committee reviews ESA applications on a weekly basis. The ESA application will be approved or declined by the ESA Committee. The student will be notified via TSU email of the ESA Committee\u2019s decision. If an ESA application is declined, the email will include suggestions made by the ESA Committee.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Submission of an emotional support animal\u00a0application, provider documentation, animal health records, and supporting materials does not guarantee a specific review timeline by the Emotional Support Animal\u00a0Committee or completion of the review process within a particular timeframe.
