Tarleton State University is now partnering with Med+Proctor for the submission of bacterial meningitis vaccination records. You are not <\/strong>required to pay a fee if you choose to \u201cSubmit your health forms for free without the upgrade<\/strong>\u201c. You may also choose to pay $10 if you would like the extra benefits Med+Proctor provides.<\/p>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n

Beginning in May 2026, for the months of May through August, all students will be required to pay the $10 upload fee.<\/strong><\/p>\n\n\n\n

  1.  You must receive the bacterial meningitis vaccination at least 10 days prior to the start of the semester.<\/li>\n\n\n\n
  2. The vaccination must be no more than five years old from the date you first enroll.<\/li>\n\n\n\n
  3. Upload a picture of your vaccination record or exemption form in the portal by following the instructions<\/a> below.<\/li>\n<\/ol>\n\n\n\n
