Applications to the Teacher Education Program

DO NOT WAIT UNTIL THE DEADLINE DAY TO SUBMIT APPLICATIONS.  NO APPLICATIONS WILL BE ACCEPTED LATE.

Instructions to apply or reapply and the order applications should be submitted in can be found below.

 

 

Applying to the Teacher Ed Program for the FIRST TIME.

Interdisciplinary Studies / Elementary - EC-6, 4-8, Special Education

1.    Fill out the first page of the application – “Interdisciplinary Studies – Request for Degree Evaluation, Certificate Plan, and Updated Degree Plan”. This page is turned into the Administrative Assistant in Room 320 of the Education Building as soon as possible.  
2.    Watch for signs in the Education Building with instructions on how to sign up for your departmental screening interview and writing.  
3.    Make an appointment with your advisor – check their office door for a list of students.  
4.    Complete the rest of the pages in the application, including the written responses and requesting letters of reference.
5.    When you meet with your advisor, make sure you obtain a signed checklist, certificate plan acknowledgement/responsibility form, degree evaluation, and test scores.
6.    Bring your completed application, along with a $35 application fee (cash, check, or money order), to Room 101 of the Math Building. PLEASE DO NOT WAIT UNTIL THE DEADLINE DAY!!!

Application Packet consists of:
The following items can be found in the Elementary Teacher Education Program Application except for the items noted that come from your advisor.

Turn into Math 101 by due date:

•    $35 Application Fee (cash, check, or money order)
•    ELEM 1 - Application (name/address/etc.)
•    ELEM 2 - Checklist
•    Certificate Plan Acknowledgement and Responsibility Agreement  (from ADVISOR)
•    Degree Evaluation   (from ADVISOR)
•    ELEM 3 – Current Enrollment Form
•    ELEM 4 – Acknowledgements
•    Test Scores

Turn in at time of departmental screening:

•    Departmental Screening Documents Form
•    2 written responses
•    3 letters of reference


Advisor signatures are required on:

•    Checklist
•    Certificate Plan Acknowledgement and Responsibility Agreement


 Secondary/All Level – 6-12, 7-12, 8-12, All Level Art, Music, PE, Spanish


1.    Make an appointment with your advisor and make any necessary arrangements to complete the departmental screening.
2.    Complete the application.
3.    Print out your unofficial transcript from myGateway and use it to complete your certificate plan.
4.    When you meet with your advisor, make sure your advisor signs your checklist and certificate plan, completes the verification of successful completion of departmental screening form or makes arrangements to complete it at a later date, and completes any necessary substitution forms.
5.    Bring your completed application, certificate plan, transcript, and departmental screening form, along with a $35 application fee (cash, check, or money order), to Room 101 of the Math Building. PLEASE DO NOT WAIT UNTIL THE DEADLINE DAY!!!

•    Application
          SEC 1 – name/address
          SEC 2 – certification area
          SEC 3 – checklist
          SEC 4 – departmental screening form
          SEC 5 – current enrollment information

•    Certificate Plan
   
•    Transcript printed from myGateway  (make sure all transfer work is showing)

•    Test Scores
    
•    Copy (not original) of New Substitution Forms (if needed)  (route original for signatures; only include a copy with student and advisor signature in file)

•    Departmental Screening Form
•    Substitution Form (if needed)
