What is the purpose of the certification form (TSU-CT/VA FORM 1)?

This form (TSU-CT/VA Form 1) serves as your permission to the VA Certifying Official to certify the veteran's enrollment for the semester. Once the classes are verified against the degree plan, the certification data is sent to the processing center in Muskogee, Oklahoma. Correct information on this form is critical to prevent a delay or mistake in your payment. A copy of the form may be obtained outside the VA Office (room 145).
