Protocol for registering with SDS
After you have applied for admission, you should get in touch with Student Disability Services to request accommodations. You will be asked to provide documentation to support your request. Appropriate documentation includes assessment results from a psychologist, psychiatrist, diagnostician, neurologist, or other medical doctor (for a physical disability or chronic medical condition).
You will meet with the Director to complete paperwork and to discuss your individual needs after submitting your documentation.
In order to utilize your approved accommodations, you will request accommodation letters each semester by completing a form and returning it to SDS. Upon receipt, staff will generate a letter for each course for which you have approved accommodations.
It is your responsibility to deliver the accommodation letters to the faculty teaching your courses. At that time, you are encouraged to take the opportunity to discuss your individual needs.
