Accommodation Letter Request Form
Each semester the student is responsible for filing this form with the Office of Student Disability Services. A student will not receive accommodation letters until this form has been completed and submitted.
ADHD Verification Form
Student Disability Services requires current, comprehensive documentation of ADHD in order to determine appropriate services and accommodations based on functional limitations that may impact the student in the academic setting. This form has been developed to assist the student in working with the treating/diagnosing professional(s) in obtaining specific and necessary information to evaluate requests for academic assistance based on the ADHD diagnosis.
