Offer Patient Assistance for their Medications.

• If you don't know the company who manufactures your medication, you can look it up on the internet.
• If an application is available online, please print it and have it filled out before you come to the Health Center.
• Please bring any required documentation, e.g. last years tax return or 2 months of pay stubs.
