Student Cohort Declaration of Intent 

I __________________ intend to participate in the MBA/CIS (circle one) cohort program beginning in spring 2009. 

-- I understand I must go to Room 146 and complete an application for Graduate School.

-- I understand I must take the two required courses each term to remain in the cohort. 

-- I understand I must complete a Cohort Scholarship Application Form and take it to room113.

-- I understand I will receive a $750.00 scholarship each term I am a member of the cohort.

-- I acknowledge I will no longer be a member of the cohort and will cease to receive the 

    scholarship funds if for any reason I fail to take the two required courses per term or fail to

    maintain a 3.0 GPA. 

-- I acknowledge that I may be required to take leveling courses for the MBA cohort.

A mandatory cohort orientation will be held on Saturday November 22.

Courses will meet five Saturdays each semester. The first class will meet from 8:00AM until 12:45PM while the second class will meet from 2:00PM until 6:45PM.

The spring 2009 cohort courses will meet on the following Saturdays:  January 31, February 28, March 28, April 25, and May 9.   Each course will be supplemented with additional online instruction.
