Applications to the Teacher Education Program

DO NOT WAIT UNTIL THE DEADLINE DAY TO SUBMIT APPLICATIONS.  NO APPLICATIONS WILL BE ACCEPTED LATE.

Instructions to apply or reapply and the order applications should be submitted in can be found below.

 

 

Applying to the Teacher Ed Program for the FIRST TIME.

Elementary - Interdisciplinary Studies – EC-6, 4-8, All Level Special Ed

  1. Fill out the first page of the application – “Interdisciplinary Studies – Request for Degree Evaluation, Certificate Plan, and Updated Degree Plan”.  This page is turned into the Administrative Assistant in Room 320 of the Education Building as soon as possible.
  2. Watch for signs in the Education Building with instructions on how to sign up for your departmental screening interview and writing.
  3. Make an appointment with your advisor.
  4. Complete the rest of the pages in the application, including the written responses and requesting letters of reference.
  5. When you meet with your advisor, make sure you obtain a signed checklist, certificate plan acknowledgement/responsibility form, degree evaluation, transcript, and test scores.
  6. Bring your completed application, along with a $35 application fee (cash, check, or money order), to Room 101 of the Math Building.  DO NOT WAIT UNTIL THE DEADLINE DAY!!!  See below for what order your application should be submitted in.

 Secondary/All Level – 6-12, 7-12, 8-12, All Level Art, Music, PE, Spanish

  1. Make an appointment with your advisor and make any necessary arrangements to complete the departmental screening.
  2. Complete the application.
  3. Print out your unofficial transcript from Duck Trax and use it to complete your certificate plan.
  4. When you meet with your advisor, make sure your advisor signs your checklist and certificate plan, completes the verification of successful completion of departmental screening form or makes arrangements to complete it at a later date, and completes any necessary substitution forms.
  5. Bring your completed application, certificate plan, transcript, and departmental screening form, along with a $35 application fee (cash, check, or money order), to Room 101 of the Math Building.  DO NOT WAIT UNTIL THE DEADLINE DAY!!!  See below for what order your application should be submitted in.

REAPPLYING - Applying to the Teacher Ed Program after being denied.

Elementary - Interdisciplinary Studies – EC-6, 4-8, All Level Special Ed

  1. Fill out the first page of the application – “Interdisciplinary Studies – Request for Degree Evaluation, Certificate Plan, and Updated Degree Plan”.  This page is turned into the Administrative Assistant in Room 320 of the Education Building as soon as possible.  This page must be submitted EVERY TIME you apply to the Teacher Education Program.
  2. If you were denied based on your interview/writing or if you must complete a growth plan, watch for signs in the Education Building with instructions on how to sign up for your departmental screening interview and writing.
  3. Make an appointment with your advisor - check their door for a list of names.
  4. Complete the rest of the pages in the application except for the Reference/Acknowledgement form.  This form is only needed again if you wish to submit different references or new written responses.  If not, we will use these from your first application.  (You do not need to request these from us – they will already be in your file.)
  5. When you meet with your advisor, make sure you obtain a signed checklist, certificate plan acknowledgement/responsibility form, degree evaluation, transcript, and test scores.
  6. Bring your completed application to Room 101 of the Math Building.  If you tell us you are reapplying, you will not be asked to pay the $35 fee again.  DO NOT WAIT UNTIL THE DEADLINE DAY!!!  See below for what order your application should be submitted in.

Application Order

We would prefer that applications be submitted in the following order:

Elementary - Interdisciplinary Studies:

  • Application (contains your name and address)
  • Checklist
  • Certification Plan Acknowledgement and Responsibility Agreement paper clipped to any coursework related documents such as: Degree Evaluation, Transcript, Certificate Plan (not required), Transfer Coursework - Focus Report (not required).
  • Reference/Acknowledgement Form followed by 2 written essays and 3 letters of reference in the order they are listed on the form. If all letters are available, packet can be stapled. If any letters are missing, please use a paperclip.    (Not necessary if you are reapplying.)
  • Current Enrollment Form
  • Test Scores (if available)

Secondary/All-Level:

  • Application (contains your name and address)
  • Application p.2 (where you declare your certificate area)
  • Checklist
  • Departmental Screening Verification Form  (only necessary to submit once)
  • Certificate Plan
  • Transcript
  • Current Enrollment Form
  • Test Scores (if available)
  • Substitution forms (if applicable)
